Step 1
In-person evaluation
Fee: $275 (in-person visit)
Meet with Dr. Shiraz Farooq at our Tampa clinic. He’ll review your medical history, determine your eligibility, and issue your first set of “orders” — including smokeable flower if appropriate.
Step 2
Telehealth renewals
Fee: $50 per renewal visit
Florida law requires new orders every 70 days. We’ll schedule quick, compliant telehealth check-ins to keep you covered after first and second 70 days.
Step 3
Ongoing Support
Fee: $150 (in-person visit)
Every 210 days (roughly 7 months), you’ll need a full recertification visit. We’ll remind you before your certification expires.

Dr. Farooq will evaluate your medical history and determine if you qualify. If so, your information will be entered into Florida’s Medical Marijuana Use Registry (MMUR).
✅ Step 3 — Apply for Your MMUR ID Card
You’ll receive an email from the Office of Medical Marijuana Use (OMMU) with your login details. From there, you’ll complete your application and pay the $75 annual fee to the State of Florida.
